I, the registrant or legal parent/guardian, in registering with Guelph Ringette Association (GRA), agree to abide by the Association’s constitution, rules, regulations, policies, principles and philosophies and those as outlined in the Ringette Ontario's Operating Manual. I agree to hold harmless the City of Guelph and the Guelph Ringette Association, their officers, instructors, officials and participants from any and all claims in respect of any damage or injury arising from participation in the program.
In the case of sickness or injury on the ice or travelling to and from games, I give permission for the registered participant to be taken to a hospital or doctor and be administered any treatment necessary. I also agree that the participant is responsible for his/her own medical coverage. Attempts will be made to contact parent or guardian first.
GRA collects personal identifiable information, such as your e-mail address, name, home address and telephone number to operate our organization, communicate with you and deliver the services you have requested. View our Privacy Statement here.
Further, I consent to the release of photographs of myself and/or the minor child or children that are registered, for use in the GRA’s print, on-line (including website and social media) and video-based marketing materials, as well as other GRA publications.
Photos may also be shared with public media to announce team activity and success with the community (i.e. team photos). Participants, volunteers or parents/guardians will not receive financial compensation of any type associated with published images.
